guardianship of elderly father

I am guardian of my father who is in an Alzheimers facility. Do I have the legal power to put restrictions on who visits him? If so, how do I do that? Do I have to create an addendum to the guardianship papers, go before a judge, and have the judge accept/decline that? If so, do I need a lawyer to write the addendum? Or can I write a letter explaining the restrictions to the facility where he resides?

You should start by carefully reading your orders. You may already have the authority. If not, then yes, you need to return to court and request the authority.
