what is the difference if any, between a state appointed attorney and a state "paid for" attorney

I think you may be referring to the fact that you may have an appointed public defender or a regional counsel, both of which are usually full time state employees, or you may have a private attorney that is being paid to handle your particular case by the state on a per case basis.
