"You need to draft and file a QDRO. This can be a document with very technical wording that needs to be cleared by the company controlling your ex-husband's retirement. Once the company approves the order, you then submit it to the court."

Thank you for your response...that being said, is this something that I can do on my own..is there a specific format? Again, I am not in the best of financial positions right now so any advice on the easiest/most affordable way to proceed is appreciated...

Each financial institution requires their own format. Again, contact the company that controls your ex-husband's retirement and ask them for assistance.
