On October 7, 2013, the Respondent was operating elevator, license number 51 940, located at Masco Contractor Services Central Inc, 2339 Beville Rd Daytona Beach, FL 32119, This elevator was inspected by State Inspector Robert J. Badke, During the inspection there were seyen viclationg noted. The violations were annual test required; fast recorded in 2007, relief valve not sealed, test jog not filled in, no code data tag in place, fire extinguisher in machine room outdated, no.emergency evacuation procedures and no phorie service in car.
