On June 2, 2015, the above clien t/applicant requested a hearing pursuant to sections 120.569 and 120.57, Florida Statutes, to contest an Agency determination. The request was transmitted to the Division of Administrative Hearings for assignment of an administrative law judge. On August 10, 2015, a Notice of Voluntary Dismissal was filed and en August 12, 2015, the Division of Administra tive Hearings closed its file and Relinquished Jurisdiction to the Agency for Persons with Disabilities.
