RECOMMENDED ORDER
Pursuant to notice, the Division of Administrative Hearings, by its duly designated Hearing Officer, K. N. Ayers, held a public hearing in the above- styled case on July 19, 1984, at Tampa, Florida.

At the commencement of the hearing, Respondent announced that she desired to surrender her license and waive further proceedings. After an explanation was given to Ms. Cox that she would thereby forfeit all rights to such license, Ms. Cox persisted in her desire to surrender her license. It is
RECOMMENDED that upon receipt of notarized statement from Margie L. Cox that she is surrendering her license a final order be entered dismissing the Administrative Complaint and closing this case.
