~ SETTLEMENT AGREEMENT
STATE OF FLORIDA, AGENCY FOR HEALTH CARE ADMINISTRATION
(‘AHCA” or “the Agency”), and D & J Medical Supply, Inc. (‘PROVIDER’), by and —
through the undersigned, hereby stipulate and agree as follows:
1. This Agreement is entered into between the parties for the purpose of _
