This is an administrative complaint by the Department of Children and Families, hereinafter referred to as the Department, against College Park United Methodist Church Child Development Center for the imposition of an administrative fine of $2,200.00. 2. College Park United Methodist Church Child Development Center, is a Child Care . Facility, licensed by the Department on license number 1184-8, issued on November 14, 2000 for operation at 644 W. Princeton St., Orlando, FL 32804 3. The Department imposes on the respondent facility one or more administrative fines, in the total amount of $2,200.00 These fines are imposed for the violations of F.S. 402.301 et. seq. and the Florida Administrative Code 65C-22, more particularly set forth in paragraph 4 of this complaint. (a) VIOLATION: The facility is in violation of FAC 65C-22.006(5) (e) and F.S. 402.301-402.319 by reason of the following facts: In March of 1999 center was advised by Certified Letter of the disqualification of staff member, Latasha (AKA Andrea) Ransom. Center rehired this staff on July 18, 2000 without an exemption been given to employee. See Inspection report of 10/24/00 attached as exhibit A. (b) For this violation, the Department imposes on the facility a fine of $2,200.00 (88 days at $25.00 per day) Section 402.310-402.312 Florida Statutes. - (a) You are hereby notified that you have the right to request an Administrative Hearing on the issue of this fine, pursuant to Section 120.56, and 120.569 Florida Statutes. You also have the right to be represented by counsel, to take testimony, to call and cross-examine witnesses and to have subpoenas or subpoenas duces tecum issued on your behalf. Rule 28-106.101 et. seq., Florida Administrative Code. qd) Your request for Administrative Hearing, if desired, must be in writing pursuant to Section 402.310 (2) and must comply with the requirements of Rule 28 — 106.101 et. seq., Florida Administrative Code. (2) Your request for Administrative Hearing must be received by the Department within twenty-one (21) days of your receipt of this complaint, pursuant to Section 402.310 (2), Florida Statutes, and Rule 28-101.111 (4), Florida Administrative Code.
