FINAL ORDER
This proceeding was initiated by The Health Center of Imperial, Inc., d/b/a Imperial
Health Care Center’s (“Petitioner”) letter of April 13, 2001 requesting a formal hearing to
contest the Agency for Health Care Administration’s (“Agency”) determination regarding the per
diem rate applicable to the Petitioner’s application for a Change of Ownership (“CHOW”).
Petitioner was notified of, the Agency’ s s decision by letter dated ‘April 3, 2001. Prior to the final
hearing i in this matter, the patties reached a » settlement i in related cases, which ¢ are identified i in the
Petitioner’s “Notice of Withdrawal of Petition”. “On June 12, 2001, Petitioner filed its Notice of
Withdrawal of Petition, and the administrative law judge ¢ entered an order of June 18, 2001
closing the Division of Administrative Hearing’ s file.
Based on the foregoing, request for a hearing is dismissed and this proceeding is closed. _
