Your business failed to demonstrate substantial compliance with the minimum standards required to be licensed. The Agency for Health Care Administration area office sent your business a certified letter on 04/19/2001 noticing you of deficiencies found during the survey conducted on 04/18/2001 and requested a Plan of Correction for those deficiencies. On 06/20/2001 the area office tried to reach you by phone and lefta message to return their _ call, however, you failed to respond to the call. On 06/25/2001 the area office sent you a second certified letter requesting the Plan of Correction and to date, an acceptable Plan of Correction has not been received. Therefore, the HME license is being denied pursuant to Section 400.931, Florida Statutes (2000) and Section 59A-25.005(1)(f), Florida Administrative Code.
