FINAL ORDER
This case is before the Agency for Health Care Administration (Agency) for
sssuance of a final order. In a “Notice of Intent to Deny” dated February 27, 2002, the
Petitioner was informed of the Agency’s initial decision to deny their renewal
application.
FINDINGS OF FACT
L. The Petitioner received the decision letter and requested an administrative
hearing. Subsequently, the Petitioner filed a “notice of voluntary dismissal” withdrawing
its request for an administrative hearing. The Division of Administrative Hearings
(division) closed his file based upon the Petitioner’s withdrawal of its request for an
administrative hearing. The division’s order of May 13, 2002, and the Petitioner’s “notice
of voluntary dismissal” are incorporated by reference.
CONCLUSIONS OF LAW

BASED on the foregoing, the request for a formal hearing is dismissed, this
