ADMINISTRATIVE COMPLAINT
COMES NOW, the Petitioner, Department of Health, hereinafter referred to as “Petitioner”,
and files this Administrative Complaint before the Board of Dentistry against, LAMONT
CANADA, D.D.S., referred to as "Respondent", and alleges:
1. Effective July 1, 1997, Petitioner is the state agency charged with regulating the
practice of Dentistry pursuant to Section 20.43, Florida Statutes, Chapter 456, Flocda Statutes,
(formerly Chapter 455, Part U; see Chapter 2000-160, Laws of F lorida), and Chapter 466, Florida
Statutes. Pursuant to Section 20.43(3)(g), Florida Statutes, the Department has contracted with
the Agency for Health Care Administration to provide consumer complaint, investigative, and
prosecutorial services required by the Division of Medical Quality Assurance, councils, or
boards, as appropriate.
2. Respondent is, and has been at all times material hereto, a licensed dentist in the State of
Florida, having been issued license number DN 9563. Respondent's last known address is 2401
Executive Plaza Road, Pensacola, Florida 32504.
3. On or about October 12, 2000, a Final Order was issued in AHCA case number 1999-
00225.
4. The Final Order in AHCA case number 1999-00225 required the Respondent to
complete eight hours of continuing education in diagnosis and eight hours of continuing education
in risk management within one year of the issuance of the Final Order. The Final Order further
required that the Respondent provide certification to the Board of Dentistry that he had completed
the required hours of continuing education.
5. As of December 10, 2001, the Respondent had failed to provide the required
certification to the Board of Dentistry that he had completed the required hours of continuing
education.
6. On or about January 10, 2002, the Respondent provided documentation to the agency
that he had completed fifteen hours of continuing education.
7. On or about January 14, 2002, Allied Health Compliance Unit (Compliance) informed
the agency that the Chairman, Board of Dentistry, had reviewed the continuing education
certification that the Respondent had submitted. Compliance stated that of the fifteen hours of
continuing education certification that the Respondent submitted, only 2.5 hours of continuing
education in diagnosis were acceptable.
8. As of December 10, 2001, the Respondent failed to complete 5.5 hours of acceptable,
certifiable continuing education in diagnosis and failed to complete 8 hours of acceptable,
certifiable continuing education in risk management.
9. The Respondent has failed to comply with the Final Order issued in AHCA case
number 1999-00225.
10. Based on the foregoing, the Respondent’s license to practice dentistry in the State of
Florida is subject to discipline pursuant to Section 466.028 (1) (aa), Florida Statutes (2001), for his

violation of a lawful order of the board that had been previously entered against him on October
12, 2000. .
WHEREFORE, Petitioner respectfully requests the Board of Dentistry enter an order
imposing one or more of the following penalties: revocation or suspension of the Respondent's
license, restriction of the Respondent's practice, imposition of an administrative fine and costs,
issuance of a reprimand, placement of the Respondent on probation, and/or any other relief that the
Board deems to be appropniate.
