On June 15, 2004, while holding a Florida insurance license, you, SILVIA AZA, pleaded nolo contendere to one felony count of grand theft in the first degree, and one felony count of intent to defraud. 7. You, SILVIA AZA, had adjudication of guilt withheld and were sentenced to five (5) years probation on each count. As a special condition of probation, you were ordered to immediately surrender any and all licenses issued by any agency of the state of Florida. IT IS THEREFORE CHARGED that you, SILVIA AZA, have violated or are accountable under the following provisions of the Florida Insurance Code and Rules of the Department of Financial Services which constitute grounds for the suspension or revocation of your licenses and appointments: (a) The licensee if an individual, or the partners if a partnership, or owner if a sole proprietorship, or the officers if a corporation, having been found guilty of or having pleaded guilty or nolo contendere to a felony or a crime punishable by imprisonment of 1 year or more under the law of the United States or of any state or under the law of any other country which involves moral turpitude, without regard to whether a judgment of conviction has been entered by the court having jurisdiction of such cases. Department records indicate that Genesis Title Services, Corp. is located at 7220 North West 36” Street, Suite 303A, Miami, Florida 33166. Claudia Camarillo, an investigator for the Department of Financial Services, visited this address and discovered that your agency was no longer at the location. 10. According to records from the Florida Department of State, Division of Corporations, Genesis Title Services, Corp. changed its address on April 28, 2004, to 792 Rio Vista Drive, Miami, Florida 33166. 11. You, SILVIA AZA, as president and sole officer and director of Genesis Title Services, Corp., failed to notify the Department in writing of the change in your principal business street address. IT IS THEREFORE CHARGED that you, SILVIA AZA, have violated or are accountable under the following provisions of the Florida Insurance Code and Rules of the Department of Financial Services which constitute grounds for the suspension or revocation of your licenses and appointments: (a) Every licensee shall notify the department in writing within 60 days after a change of name, residence address, principal business street address, or mailing address. Any licensed agent who has moved his or her residence from this state shall have his or her license and all appointments immediately terminated by the department.
