MAR=14-2486 13:33 DCF LICENSING OFFICE 447 228 6432 STATE OF FLORIDA DEPARTMENT OF CHILDREN AND FAMILIES IN THE MATTER OF A Civil Penalty Against Community Coordinated Care for Children, Inc. Park Avenue Headstart 301 South Oak Ave. Sanford, Fl. 32771 ADMINISTRATIVE COMPLAINT YOU ARE HEREBY NOTIFIED that the Department has imposed a Civil Penalty in the amount of $200.00. As grounds for the imposition of this penalty, the Department states the following: 1. The Department of Children and Families, State of Florida, has jurisdiction over Respondent by virtue of the provisions of sections 402,301- 402.319, Florida Statutes. 2. The Respondent, Community Coordinated Care for Children, Inc., dba Park Avenue Headstart, was licensed on license #C07SE0146, issued September 15, 2005 to operate Park Avenue Headstart Family Center —301 South Oak Avenue, Sanford, FI. 32771, as a Child Care facility in accordance with Chapter 402, Florida Statutes, and Florida Administrative Code, Rule 65C-22. 3. Respondent committed violations of Rule 65C-22.002(7)(c) with repect to Monthly Fire Drill and Rule 65C-22.0065(5)(d), Section 435.04, Florida Statutes, with respect to Form 5131/Screening Documents. During inspection of December 15, 2005 the facility was cited for violations of Monthly Fire Drill and Form 5131/Screening Documents with a due date of December 31, and December 30, 2005 respectively, On January 26, 2006 the facility was sent an Intent to Impose Administrative Action for failure to comply with these items with a due date of February 10, 2006. As of February 11, 2006 the facility has not complied with either of these items. The inspection report and the Intent to Impose Administrative Action have been submitted as "Exhibit A.” For failure to comply and follow through with Monthly Fire Drill and Form 5131/Screening documents, the Department imposes an administrative fine of $200. This fine is imposed in accordance with Section 402.310(1)(a), Florida Statutes. 4. The above referenced violations constitute grounds to levy this Civil Penalty: pursuant to s. 402.310(1)(a), F. S., in that the above referenced conduct of Respondent constitutes a violation of the minimum standards, rules and regulations for the operation of a Child Care Facility.
