On April 30, 2009, a Notice of Intent to Deny Licensure was served upon Nacional Memorial Funeral Chapel, LLC. Pursuant to Section 120.57(1), Florida Statutes, Petitioner requested a formal hearing and the case was referred to the Division of Administrative Hearings on June 8, 2009. After conducting several depositions the Petitioner withdrew its Petition for formal hearing from the Division of Administrative Hearings and an Order Closing the File was issued on July 20, 2009, by Administrative Law Judge, John G. Van Laningham. Accordingly, IT IS HEREBY ORDERED that the application is DENIED.
