ADMINISTRATIVE COMPLAINT
Comes now, the Division of Alcoholic Beverages and Tobacco, herein after referred to as "petitioner," and files this administrative complaint against WILSON, JEANINE MICHELE herein after referred to as "respondent,"and alleges:
Petitioner is the state agency charged with supervising businesses licensed under the Florida beverage, tobacco and tax, and tobacco products permit laws pursuant to sections 559.061, 561.07, 561.15, 561.19, 561.29, 561.501, 210.15, 210.16, 210.45, 210.50, 569.003, and 569.006, Florida Statutes.
Respondent is, and has been at all times material hereto, the holder of a valid license/permit, issued in the State of Florida, by the Division of Alcoholic Beverages and Tobacco. Respondent's last known mailing address is as follows:

And the division has not been notified of any other designated person or address for the service of process and notices authorized by Rule 61A-2.012, Florida Administrative Code.
Petitioner intends to revoke; suspend; annul; impose civil penalties, investigative costs, and late penalties; or any combination of these authorized penalties. This intended action is based on the below described facts or conduct.

You, WILSON, JEANINE MICHELE, D/B/A CHASES BEER & BAIT, are charged with the following count(s):
On or about 05/02/2012 and 06/01/2012, you the aforementioned licensee, holding a license under the Beverage Law, or your agent(s), officer(s), servant(s), or employee(s) or other person, to wit: (Lisa Anderson), did unlawfully on your licensed premises, allow or permit the possession of alcoholic beverages, to wit: a single Jello- Shot (Vodka), 56 Jello-Shots (Vodka),1-750 ml bottle of Skol,1- 750 ml bottle of Captain Morgan,1- 750 ml bottle of Wolfschamidt Vodka, and 1 clear jar/ unknown Liquor (16oz) not authorized by law to be sold under your license, this act being contrary to and in violation of Section 562.02, within 561.29(1)(a), Florida Statutes.
On or about 04/25/2012, 04/27/2012, and 05/02/2012, you, the aforementioned licensee, holding a license under the Beverage Law, or your agent(s), officer(s), servant(s), or employee( Lisa Anderson), did unlawfully on your licensed premises, permit or allow gambling activity, game(s) of chance for money or other thing of value, to wit: Dice game by rolling(5) dice in a clear plastic jug for twenty-five (.25 cents) per play with a chance of winning prize money which did not require any skills. This act being contrary to and in violation of section 849.11, within 561.29(1)(a), Florida Statutes.
