On August 25, 2015, the above applicant requested a hearing to contest an Agency determination. The request was transmitted to the Division of Administrative Hearings for assignment of an administrative law judge. On December 28, 2015, a Notice of Voluntary Dismissal was filed and on January 4, 2016, the case was dismissed. The Division of Administrative Hearings closed its file. A copy of the Order Closing File is attached and incorporated herein.

It is, therefore, ORDERED that this case is DISMISSED. The Agency's file in this matter is closed.
