FINAL ORDER
THIS CAUSE came on for consideration before the Agency for Health Care Administration (" the Agency" ), which finds and concludes as follows:
The Agency issued the Petitioner the attached Notice of Intent to Deem Change of Ownership Application Incomplete and Withdrawn from Further Consideration. (Ex. 1).
The parties have since entered into the attached Settlement Agreement. (Ex. 2) Based upon the foregoing, it is ORDERED:
The Settlement Agreement is adopted and incorporated by reference into this Final Order.
The parties shall comply with the terms of the Settlement Agreement.
The Petitioner' s application for licensure is voluntarily withdrawn.

NOTICE OF RIGHT TO JUDICIAL REVIEW
