STATE OF FLORIDA DEPARTMENT OF CHILDREN AND FAMILIES Pine Hills Christian Child Care 800 N Pine Hills Rd, Orlando FL, 32308 Certified: Mail Return Receipt No.7 OF /OCOO0FF0bb070 ADMINISTRATIVE COMPLAINT YOU ARE HEREBY NOTIFIED that the Department has imposed a Civil Penalty in the amount of $90.00 and six (6). months probation, The Department’s authority and grounds to impose this sanction are explained below. 1, The Department-of Children and Families is authorized by section 402.310, Florida Statutes, to sanction Pine Hills Christian Child Care for violations of child care licensing standards in sections 402.301- 402.319, Florida Statutes, and chapter 65C-22, Florida Administrative Code. Pine Hills Christian, Child Care is licensed under chapter 402, Flotida Statutes, and chapter 65C-22, Florida Administrative Code, to operate a child care facility located at 800'N Pine Hills Rd, Orlando FL, 32808. The facility license is currently a Regular License. Respondent committed aClass II licensing violation of 65C.002(4)(c), Florida Administrative Code, with respect to the Outdoor Play Area Rule. On May 6, 2015, and January 25, 2017; the facility’s outdoor play area was observed to pose a threat to the health, safety or well-being of the children due to the presence of hazardous items. The inspection is attached as Exhibit “A”. The foregoing facts violate 65C.002(4)(c), Florida Administrative Code. The violation described above is a Class II violation of child care licensing standards. It is the facility’s second Class II violation of the Outdoor Play Area Rule within a two-year period, For this violation, the Department imposes a $50 fine which is imposed in accordance with Section 402.310(10)(a), Florida Status. And 65C-22.010(2)(e)1, Florida Administrative Code. Respondent committed a Class Il licensing violation of 65C-22:006(4)(c), Florida Administrative Code, with respect to the Personnel Records Rule. On January 26, 2015, September 4, 2015, January 6, 2016, April 26, 2016 and January 25, 2017; the personnel record did not include-a signed CF- FSP 5337 Child Abuse and Neglect Reporting Requirement form. The inspections are attached as Exhibit “A”. The foregoing facts violate 65C-22.006(4)(c), Florida Administrative Code. at 8. The violation described above is a Class II violation of child care licensing standards. It is the facility’s fifth Class I violation of the Personnel Records Rule within a two-year period. For this violation, the Department imposes a $40 fine which is imposed in accordance with Section 402.310(10)(a), Florida Status. And 65C-22.010(2)(e)1, Florida Administrative Code. 9. Asa result of the above violations, Pine Hills Christian Child Care is hereby placed on Probation for a_period not to.exceed six months. During:this term of probation the respondent shall be required to meet the following terms of probation: *Personnel Records* Total due: $90 If you do not wish to contest the findings of this administrative complaint, payment of the fine amount may be made directly to the Florida Department of Children and Families, Child Care Regulation Office by money order or cashier’s check.
