FINAL ORDER

THIS CAUSE comes before the STATE OF FLORIDA, AGENCY FOR HEALTH CARE

ADMINISTRATION ("Agency") concerning the denial of a request for an exemption from disqualification from employm ent /Medicaid provider enrollment.

I. On August 17, 2020, the Agency issued Petitioner a letter denying his application

for exemption from disqualification for emp loyment and/or from enrollment as a Medicaid provider (attached as Exhibit A).

1. On September 1 , 2020, Petitioner ft le.d a Petition for an Administrative Hearing with respect to the denial of his application for exemption from disqualiftcalion for employment (attached as Exhibit B).

2. On March 1 0, 2021, the Division of Administrative Hearings ( " DOAH " ) issued an Order Requiring Status Report stating that the issue raised in the Petition may be moot, as the Department of Health (DOH) entered a Final Order revok i ng the Petitioner·s li cense to practice psyc holo gy (attached as Exhibit C).

3. On March 15, 2021, the parties filed a Joint Status Report and Motion to Relinquish Jurisdiction (attached as Exhibit D).

4. Also on March 15, 2021. DOAH issued an Order Relinquishing Jurisdiction and Closing File (attached as Exhibit E).

   Based on the foregoing. it is therefore ORDERED and ADJUDGED that:

   
   

   1. The Petitioner's request for a formal hearing is now moot;

1. The Petition is dismissed; and

   
   

2. The above-styled case is CLOSED.
