A i es, Bia. iw . led 4. Respondent's last known address is 3614 Landings Way, Apartment #10, Tampa, Florida 33624. 5. An inspection was performed by agents of the Petitioner. As a result, it was discovered that on May 24, 1999 the Respondent misfilled a prescription refill submitted by G.G. for S.S., for the medication Effexor 37.5 mg., by dispensing Effexor 75mg. 6. On July 27, 1999, the Respondent admitted to misfilling the prescription on May 24, 1999 for S.S.. 7. The conduct of the Respondent as aforesaid is contrary to the provisions contained in Section 465.016(1)(e) and 465.016(1)(g), Florida Statutes by using in the compounding of a prescription, or furnishing upon prescription, an ingredient or article _ different in any manner from the ingredient or article prescribed, except as authorized in s. 465.019(6) or 465.025.
