BUTLER, RESPONDENT. / po-aloe AMENDED ADMINISTRATIVE COMPLAINT COMES NOW, the Petitioner, Department of Health, hereinafter referred to as "Petitioner", and files this Administrative Complaint before the Board of Dentistry against, D.D.S., referred to as "Respondent’, and alleges: 1. Petitioner is the state agency charged with regulating the practice of dentistry pursuant to Section 20.43, Florida Statutes; Chapter 455, Florida Statutes; and Chapter 466, Florida Statutes. In or around December 1996, Patient C.M. presented to a subsequent treater who informed her that the crown on tooth number fourteen (14) required additional work due to an open distal margin. 5. Inor around October 1998, an Agency expert examined Patient C.M. The Agency’s , expert opined that there was an open distal margin of the crown on tooth number fourteen (14.) 6. Based on the foregoing, the Respondent's license to practice dentistry in the State of ...
