Respondent is, and has been at all times material hereto, having been issued license number PS 0031609. a licensed pharmacist, VE WU WU 4. Respondent’s last known address is 3322 Bishop Park Drive, #617, Winter Park, FL 32792. 5. At all times material hereto Respondent is employed by Eckerd Drugs, 2652 LK . Howell Lane, Winter Park, FL 32792. 6. On or about October 13, 1997, Patient C.T. was prescribed 5 mg Ritalin tablets. 7. On or about October 13, 1997, Respondent furnished Ritalin tablets. Patient C.T. with 20 mg 8. On or about October 13, 1997, the label on the prescription furnished by Respondent indicated that the medication was 5 mg Ritalin tablets. 9. Based on the forgoing, the Respondent’s license to practice pharmacy in the State of Florida is subject to discipline pursuant to Section 465.016(1)( furnishing upon prescription, an ingredient or article different in any m or article prescribed. t...
