ADMINISTRATIVE COMPLAINT
COMES NOW, Petitioner, DEPARTMENT OF HEALTH, by and through its
undersigned counsel and files this administrative complaint before the Board of Nursing -
against the Respondent, LOGAN TAYLOR LANHAM, and in support thereof alleges: +
1. Petitioner is the state agency charged with regulating the practice of
nursing pursuant to Chapters 20, 455, and 464, Florida Statutes.
2. Pursuant to the authority of Section 20.43(3)(g), Florida Statutes, the
Petitioner has contracted with the Agency for Health Care Administration to provide
consumer complaint, investigative and prosecutorial services required by the Division of
Medical Quality Assurance, councils or boards, as appropriate, including the issuance of
emergency orders of suspension or restriction.
3. Respondent is and has been at ail times material hereto, a licensed
REGISTERED NURSE in the State of Florida, having been issued license number RN
3221312.

5. At all times material hereto, Respondent was employed by Indian River
Memoria! Hospital.
6. On or about March 1, 1999, Respondent administered Thorazine to Patient
J.M. at approximately 3:30 p.m., by placing the Thorazine in Patient J.M.'s soft drink
(Pepsi).
7. Mixing Thorazine with Patient J.M.'s sof drink was an inappropriate
manner by which to administer the medication.
8. © On or about March 1, 1999, Respondent did not inform Patient J.M. that
he was administering the Thorazine to Patient J.M. via the soft drink.
9. On or about March 1, 1999, Respondent failed to timely document the-+- .
administration of the Thorazine to Patient J.M.
10. The minimal acceptable standards of prevailing nursing practice require
that nurses timely document the administration of medication to patients under their care.
11. The failure to timely document the administration of medication to
patients could result in harm to the patient.
Count I
12. Based on the foregoing, Respondent is subject to discipline pursuant to
section 464.018(1)(h), Florida Statutes, by unprofessional conduct by failing to conform
to the minimal acceptable standards of prevailing nursing practice, by administering
Patient J.M.'s medication in an inappropriate manner.
Count II
13. Based on the foregoing, Respondent is subject to discipline pursuant to
section 464.018(1)(h), Florida Statutes, by unprofessional conduct by failing to conform

to the minimal acceptable standards of prevailing nursing practice, failing to inform
Patient J.M. that the patient was being administered medication through the soft drink.

14. Based on the foregoing, Respondent is subject to discipline pursuant to
section 464.018(1)(h), Florida Statutes, by unprofessional conduct by failing to conform
to the minimal acceptable standards of prevailing nursing practice, by failing to timely
document the administration of the Thorazine to Patient J.M.
WHEREFORE, Petitioner respectfully request that the Board of Nursing enter an
Order imposing one or more of the following penalties: Revocation or suspension of
Respondent's license, imposition of an administrative fine, issuance of a reprimand,-.
placement of the Respondent on probation, costs, and/or any other relief that the Board
deems appropriate.
