JEAN PHILLIPS SALYER, RN. and in support against the Respo! thereof alleges: 1. Petitioner is the state agency charged with regulating the practice of nursing pursuant to Chapters 2 20, 455, and 464, Florida Statutes. 2 Respondent is and has ‘been at all times material hereto, a licensed [REOISTERED NURSE it in the State of 9f Florida having been issued license number RN 10201 62. Respondents last known address is 134 Plantation Blvd., Ft:.Pierce, | Florida 34982. os or about uly 1994, Respo ndent's license to practice nursing was the Florida Board of Nursing (hereinafter “yeferre “provide quarterly self and employer reports to sd to as “the | ae . the Board, b) notify her current employer of her probationary status and c) work only under direct supervision. 5. On or about February 1996, Respondent's probation was extended by the Board after consideration of information provided by Board staff; the direct supervision requirement was dropped from the terms of Respondent's probation. 6. After the February 1996 order was issued by the Board, the Respondent worked for an employer without notifying her sr employer that her jicense was on probation as required by the terms of her probationary order. 7. Based on the foregoing, Respondent's license to practice nursing is subject to discipline pursuant to section 464.018(1)(), by knowingly violating a lawful order of the board previously entered in a disciplinary proceeding.
