Keron BOARD OF ORTHOTISTS AND PROSTHETISTS HEATH REINHARTZ, Petitioner, vs. DOAH Case No. 00- 7€0 2 BOARD OF ORTHOTISTS SBK a AND PROSTHETISTS, 2 4 Respondent. ms 2 FINAL ORDER THIS MATTER came before the Board of Orthotists and Prosthetists (hereinafter “the Board”) at a duly-notice public meeting held on October 20, 2000, in Tampa, Florida, pursuant to the timely petition of HEATH REINHARTZ (hereinafter “Petitioner”) for a hearing in accordance with the provisions of Sections 120.569 and 120.57(4), Florida Statutes, concerning Petitioner’s application for licensure. Petitioner’s application was denied by Order of the Board filed on March 29, 2000. This matter was referred to the Division of Administrative Hearings, and assigned to Judge Susan B. Kirkland. On June 12, 2000, at the request of the parties, the matter was placed in abeyance at the Division. Petitioner appeared before the Board to present additional evidence in support of his eligibility to be licensed as a Pedorthist. On June 25, 1999, the Department of Health issued a Notice To Cease And Desist the unlicensed practice of pedorthics. Upon consideration of the information provided by the applicant, the Board has determined that the application for licensure should be APPROVED; however, because of Petitioner’s unlicensed activity, the Board imposes on Petitioner’s license the following: The license of HEATH REINHARTZ is hereby REPRIMANDED. The licensee must pay an administrative fine of $500.00 and investigative costs of $1,585.00 within six months of the Board Order. The license of HEATH REINHARTZ is placed on probation for one year. It is therefore ORDERED that the Notice of Intent to Deny is hereby vacated, and the application of HEATH REINHARTZ is APPROVED. This Order shall become effective upon filing with the clerk of the Department of Health. DONE AND ORDERED this 3l day of Ok, 2000.
