Q _AUSBORN, JR. ("Respondent") and alleges: dto the following: ; rd poe a. Onor between February 10 and 11, 1996, Respondent signed out Demerol 75 mg on 6 occasions for a patient whose physician order called for Demerol 50 mg. The Réspondent did not document wastage on any of these occasions... “D. On or between February 15 and 20, 1996, Respondent signed out Demerol _ “~“iaigott 9 occasions for patient whose physician order called for Demerol 25 mg. ; ch occasion there was Demerol 25 mg available in the narcotics cabinet. On éith occasion, the signature of the waste witnessing nurse is illegible. On three --- @Géasions; there is no documentation of any kind that medication was administered to the patient. On or about March 5, 1996, Respondent signed out 2 Demerol 50 mg : on Cc. ication of the MAR or in the ampules but did not document administering the me nurses notes. 4. Onor about March 6, 1996, Respondent signed out and documented adr -r via IV, Demerol 75 mg to a patient which had been receiving Normal Saline Placebos for pain for approximately three weeks. e, On or about march 8, 1996, the narcotic count for the North hall was correct ~ © at 8:00 p.m. The next Count should have been conducted at 11:00 pm, by the ent (the outgoing nurse) and the oncoming nurse RW.
