Effective July 1, 1997, Petitioner is the state agency charged with regulating the practice of medicine pursuant to Section 20.43, Florida Statutes; Chapter 455, Florida Statutes, and Chapter 458, Florida Statutes. Pursuant to the provisions of Section 20.43(3), Florida Statutes, the Petitioner has contracted with the Agency for Health Care Administration to provide uired by the Division of Medical oe we 2. Respondent is and has been at all times material hereto a licensed physician in the state of Florida, having been issued license number ME 0050806. Respondent’s last known address is 7918 Shenandoah Lane, Parkland, Florida 33067, 3. The Respondent is not board cetifed and practices in the area of anesthesiology. 4. On or about February 20, 1998, the Respondent applied for appointment to the - medical staff at the Kimmel Out Patient Surgical Center, West Palm Beach, Florida, claiming board eligibility in anesthesiology. 5. Respondent enclosed a certificate with her application, indicating that she was board certified by the American Board of Anesthesiology. In fact, the Respondent is neither board certified nor eligible for certification. 6. Respondent knowingly made a false representation of having board certification and provided a falsified certificate with her application to become part of the medical staff at Kimmel Surgical Center. . 7. Onor about July 15, 1999, the Respondent provided a statement through her attorney admitting to the allegations. 8. Based on the foregoing, the Respondent violated Section 458. 331(1)(k), Florida Statues by making deceptive, untrue, or fraudulent representations, in or related to the practice of medicine or employing a trick or scheme in the practice of medicine. WHEREFORE, the Petitioner respectfully requests the Board of Medicine enter an order imposing one or more of the following penalties: permanent revocation or suspension of the Respondent’s license, restriction of the Respondent’s practice, imposition of an administrative | Se fine, issuance of a reprimand, placement of the Respondent on probation, the assessment of costs related to the investigation and prosecution of this case as provided for in Section 455.624(4), Florida Statutes, and/or any other relief that the Board deems appropriate.
