RESPONDENT.
Petitioner, Department of Health, hereinafter reférred to as “Petitioner”, files this
Administrative Complaint before the Board of Dentistry| against STEVEN GLICKMAN,
D.D.S., hereinafter referred to as “Respondent”, and alleges:
1. _- Petitioner is the state agency charged with regulating the practice of
dentistry pursuant to Section 20.43, Florida Statutes and| Chapters 455 and 466, Florida
Statutes. Pursuant to the authority of Section 20.43 (3)(g), Florida Statutes, the Petitioner
has contracted with the Agency for Health Care Administration, (hereinafter referred to
as the “Agency,” to provide consumer complaint, investigative, and prosecutorial .
services required by the Division of Medical Quality Assurance, councils or boards, as
appropriate, including the issuance of emergency orders of suspension or restriction.
2. Respondent is, and has been at all times material hereto, a licensed dentist
in the State of Florida, having been issued license number DN 12627. Respondent’s last
known address is.7301A, W. Palmetto Park Road, Boca Raton, Florida 33434.
3. The Respondent first examined Patient J.F. on or about November I,
1993. : _
4. _ Patient J.F. had a history of periodontal dipease before presenting to the
Respondent on or about November 1, 1993.
5. When the Respondent examined Patient JJF in November 1993, the
Respondent did not examine the patient’s complete mouth. .
6. When the Respondent examined Patient JIF., on November 1, 1993,
the Respondent developed dental and medical history records justifying the course of
treatment for the restoration of the patient’s lower teeth anly.
7. The Respondent examined Patient J.F. onjor about March 6, 1996.
8. When the Respondent examined Patient JIF. on or about March 6, 1996,
the Respondent did not examine the patient’s complete mouth.
9. On or about March 6, 1996, the Respondent examined the patient and
developed a treatment plan justifying the course of treatrnent for the patient’s upper teeth
only.
10. Patient J.F. did not receive treatment for His periodontal disease from the
Respondent during the time the Respondent treated the patient for the restoration of his
lower teeth during 1993.
11. _ Patient J.F. did not receive treatment for his periodontal disease from the
Respondent during the time the Respondent treated the patient for the restoration of his
upper teeth during 1996.
12. The Respondent’s failure to examine the patient’s complete mouth in
1993, and the Respondent's failure to develop dental and medical history records

justifying the course of treatment for the patient’s periodontal disease for both the
patient’s upper and lower teeth resulted in the unsuccessful treatment of the patient’s —-
lower teeth by the Respondent.

13. Petitioner realleges and incorporates by seference the foregoing
allegations as if fully stated herein. .
14. _ Based on the foregoing, the Respondent’s license to practice dentistry in
the state of Florida is subject to discipline pursuant to Section 466.028(1)(m), Florida
Statutes, for failing to keep written dental records and mfdical history records justifying
the course of treatment of the patient including, but not limited to, patient histories,
examination results, test results, and X-rays, if taken..
WHEREF ORE, Petitioner respectfully requests the Board of Dentistry to enter an
order imposing one or more of the following penalties: fevocation or suspension of the
Respondent's license, restriction of the Respondent’s practice, imposition of an
administrative fine and costs, issuance of a reprimand, placement of the Respondent on
probation, and/or any other relief that the Board deems dppropriate.
