The issues for determination are whether Respondent violated Section 458.331(1)(m) and (t), Florida Statutes (2000), respectively, by failing to keep medical records that justify the course of treatment and by failing to practice medicine with that level of care, skill, and treatment which is recognized by a reasonably prudent similar physician as being acceptable under similar conditions and circumstances.
