use of an A.K.A .form

Is there a form that can be used to change the spelling of your first name (i.e. Dione to Dyonne) so that it can be used for forms/records for schools? Someone submitted admission forms for colleges but he changed the spelling of his first name and has been using it since kindergarten. Can an A.K.A. (also known as) be signed and notorized using both spellings?

Federal Financial Aid regulations will require the college to resolve the name discrepancy. You will probably have to change it legally or revert to the legal name.

Check with the college. If you do have to change the name legally, the Clerk of Court for Special Proceedings in the County where you live should be able to help you get this done. A small filing fee is required.
