What form or doucument is needed to give another person permanent guardianship/power of attorney for a child in case somethings happens to the parent?

You can nominate a guardian. It's not that complicated, just prepare a declaration stating your intentions. Call an estate planner, s/he should be able to give your specific directions. Don't use a power of attorney. You want to simply nominate a guardian, and perhaps successor guardians.
