Does a lawsuit against a company have to be filed in the exact city & state where the incident occurred?

No. You can file suit in several different locations, depending on the facts of the case. In any case (no matter what the facts), you can always sue a defendant where he lives. In the case of companies, this means that you can sue a company wherever the company is "domiciled". Louisiana corporations must register their domicile with the Louisiana secretary of state. The information is available online on the secretary of state's website.
