clothing with lewd, vulgar comments worn into mother-baby unit
I work a mother baby unit which services many christian families with children who visit their siblings after birth. REcently many male visitors have worn clothing with very lewd messages on them. We were told we could do nothing because we would be going against their freedom of speech. Is there anything we can do to protect our mothers and babies from being exposed to this filth?

The First Amendment only bars the government from restricting speech. If you work at a private hospital, your employer (though maybe not you) can bar such clothing from the premises. Under some circumstances a public hospital could probably do so, too.
I understand your concern about the mothers and siblings, but I note that newborn babies can't read and surely won't have any memory of visitors' clothing by the time they can. They won't be offended or harmed in any way by the messages you describe.
