temporary guardianship

I am going to have my god daughter for a couple of months due to her mother having ''issues''. My question is I would like to find a form stating that I am not responsible if something happens to her daughter like injuries death and so forth...so she can't sue me if something does happen. Thank you in advance

First, an individual cannot absolve you from liability for criminal acts. Many acts leading to injuries and/or death would normally be considered criminal acts.

Second, the mother has no authority to release you from a duty of due care (duty to act as a reasonable person would under the circumstances) for the child.

In summary, all people are held liable for their actions, and unfortunately there is no such document.
