If I fell and hurt myself on somebody's property, how are the property owners liable??

The law on this issue changes from state to state. But it sounds like you're talking about what the law would define as a permanently unsafe condition (even though the condition could have been fixed). That said, the burden of proving liability of the property owner is less difficult. However, in any slip and fall case it is possible that the insurance company may claim that even though the property was in a negligent condition, that you are more to blame than the property owner/manager. You will likely need an attorney, quickly, to help you with this sort of claim. Speed is important because often there is evidence that can be preserved, by a lawyer, that may be lost if you don't hire experienced counsel quickly.
